Courteous answering of phones, receiving of all intake information and entering the appropriate information on Agency intake forms.

Dissemination of intake referrals thru proper channels as outlined in agency policies.

Keeping Agency Director/Supervisor up to date on all new cases coming into the Agency ongoing.

Documentation and reporting when there is a change in Consumer status, change in service requirement, change in staff assigned to case and reports any unsatisfactory performance.

Participation in the evaluation of field staff in regard to punctuality, reliability, assignments and related matters.

Telemarketing Agency services when appropriate.

Productive and uses time efficiently.

Follows instructions, punctual and attendance is acceptable.

Cooperates with supervisor and with Consumer/family/other staff.

Is self-reliant and plans appropriately.

Demonstrates effective communication with other agency staff.

Performs other duties as required.
